Affix talismans and spellhearts to your equipment the way the rules describe, and let the module handle the bookkeeping. Attachments appear as sub-items nested under the gear they're on, spellhearts generate a working PF2e effect with rule elements for whatever part of them can be automated, and talismans are consumed properly when used from a chat card.
Talismans are supported but don't exist on SF2e, and using them requires pf2e-anachronism.

Any talisman or spellheart in a character's inventory gets an Affix control on its inventory row. Clicking it opens a picker showing every piece of gear it can legally attach to, with occupied slots marked and existing attachments shown as badges.
Once affixed, the attachment nests visually under its gear as a sub-item on the character sheet. Spellhearts immediately generate an Effect item on the actor: description, token icon, and rule elements for the passive parts. Talismans aren't consumed until you manually trigger them.
When you roll with that gear, the chat card grows a collapsible panel offering Inspect and Use/Activate for each attachment. Using a talisman posts its card, consumes one, and detaches it. Activating a spellheart just posts its details.
Type comes from traits. A consumable with the talisman trait is a talisman; anything with the spellheart trait is a spellheart.
The picker shows every valid piece of gear as a card with icon, name and type, and each card carries badges for whatever is already affixed to it. Full items are marked and can't be selected.
The per-item limit is one attachment, raised to 2 when the actor has the Talismanic Sage feat, and the picker says so.
Affixing from a stack of 5 talismans decrements the stack and creates a single tracked copy rather than flagging all five. Stacked gear behaves the same way: affix to one of three identical swords and one splits off.
You get a warning naming the problem when the item is already affixed, when no slots remain, when no valid gear exists (it names the types you'd need), and when the item isn't in a character's inventory.
Two ways off: click the affixed item's icon on the character sheet, or use the detach control on its nested sub-item row.
Delete a piece of gear and its attachments are removed. Delete an attachment and the slot frees up, taking any spellheart effect with it.
When you roll with a piece of gear, such as during an attack roll, the chat card will feature a collapsible panel which you can click to "Inspect" or "Activate" an attached talisman. Using a talisman consumes it; information about its effects is then posted in the chat.
Affixing a spellheart gives you an effect so you can keep track of it with all the information you need. Passive effects without a clause are applied automatically, while most active effects need to be applied manually, so those are accompanied by a note/reminder.

Talismans get no rule elements at all. There are 180 of them and most are single-use activations rather than anything a persistent effect can express. Affixing one links it to the gear and lets you use it from the chat card.
A new PF2e release can add a spellheart we haven't covered. It still affixes and shows its description, and warns you it has no mechanics, until an update catches up.
Activate doesn't check the trigger. The rules say you activate a Flaming Star by casting a fire spell. The button doesn't verify that you did, so it will let you activate whenever you press it.
